To serve you in the best way possible, we work by scheduled appointments. We suggest you call prior to the time you wish to be seen for routine and annual appointments. Please let us know of any issues that indicate that you need to be seen sooner as we have same day appointments for emergencies.
To Cancel or Reschedule
If you are unable to keep an appointment, we ask that you cancel/reschedule your appointment at least 24 hours prior to your scheduled appointment time in order to avoid a missed appointment charge. We greatly appreciate this consideration.
Insurance Plan Participation
At Danbury Midwifery Group, we participate with most insurance plans but suggest that you check with your insurance company to verify our participation in your specific plan.
At your initial visit, you will be asked to present your insurance card, photo ID, and to complete new patient paperwork. After scheduling your first appointment you will be mailed the necessary new patient forms or you may download the necessary forms here. To save time during your first appointment, please complete the new patient paperwork ahead of time and bring it with you to your appointment.

You are responsible for learning details of your coverage from your insurance plan, including all deductibles, co-payments and/or any referrals that may be required by your insurance company. Your co-payment is expected at the time of service, and payment in full is expected at each visit for services not covered by your insurance. Additionally, if you do not have an insurance card available, and/or our providers do not participate with your health plan, full payment also is expected at the time of service.
For your convenience, we accept cash, check, Mastercard, VISA, Discover and American Express. All returned checks will be assessed a $25.00 service charge. If you are an existing patient with a balance due, you can also pay your bill online using the Patient Portal.
